Please click the link below to download, complete and print the OPRA form for submission.  Once you have completed the requestor and record request information on page 1, you may submit the form the following ways:  

1. Drop off at the front window in the lobby of the Lower Twp. Police Department,
   at 405 Breakwater Road in Erma, between the hours of 8 a.m. and 4 p.m.
2. Scan and email the completed form to opra@lowertownshippolice.com
3. Fax the completed form to 609-886-5289
